End-of-Life Doula Training: Participant Application

Please respond to the following questions with thoughtful reflection. Please do not respond with one or two-word answers. Admission to this program is generous, so you don’t need to worry that we will not select you. The purpose of this application is for you to reflect on your reasons for joining this training, what you would like to get from it, and how you will use the information and skills you develop. It also helps us to get to know you a bit better.

After we have reviewed your application, you will receive a formal invitation to join the next cohort. Please check your calendar before applying as YOU MUST ATTEND ALL SESSIONS OF THE TRAINING.
